​The Chronic Anticoagulation Management Program (CAMP) provides patients who must manage their blood levels with the use of Coumadin or Warfarin, daily support through consistent medication monitoring and/or adjustment, and reliable communication for the management of their health conditions. Patients are contacted to review medication, nutrition, falls prevention and overall health to ensure patients remain with positive health outcomes.
