Teleophthalmology Screening

This is a mobile program that provides an integrated approach to diabetes management, by screening patients at risk for retinopathy (eye condition) within their home communities. 

This program facilitates timely access to treatment and connecting patients to other members of the healthcare team.

Patients learn self-management skills from a team of health care professionals who will provide patients with the information, skills and confidence they need to manage diabetes and lead a healthy life.
